Trinity Health PRN/Contingent After-Hours Hospice RN in Livonia, Michigan

On call coverage area Wayne/Washtenaw/Livingston/Oakland Counties based on needs of department

Hours: 5pm-8am

Coverage Area: Livingston/Oakland/Washtenaw/Wayne Counties

Position Purpose:

The After Hours Hospice RNs is fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes, is designated as the case manager for hospice patients. The Hospice RN in collaboration with the physician and other Hospice team members anticipates and manages patient’s symptoms, assists in identifying pt/family/caregiver grief/loss issues and assists in implementing the Hospice plan of care. Provides functional support to Hospice Aides and LPN/LVNs as needed.

Patient Care :

  • Provide on-call nursing care during evenings, nights, weekends, and holidays.

  • Respond promptly to patient calls and after-hours emergencies, assessing and managing pain, symptoms, and comfort.

  • Perform home visits or facility visits when necessary to provide patient care and assess needs.

  • Offer emotional support to patients and families, ensuring they understand changes in care plans or conditions.

Clinical Collaboration :

  • Coordinate care with interdisciplinary team members, including physicians, social workers, and chaplains, to ensure continuity of care.

  • Communicate patient conditions and any changes with the hospice team and physician, adjusting care plans as needed.

Documentation :

  • Complete all required clinical documentation accurately and in a timely manner according to hospice standards and regulatory requirements.

  • Maintain up-to-date records of all after-hours visits, calls, and patient status changes

Minimum Qualifications:

  • Associate's or Bachelors degree in Nursing from accredited program.

  • Current active RN license with the State of Michigan

  • Minimum 1-2 years of hospice, palliative care, or home health experience.

  • Must have current Driver’s license and reliable transportation to and from work site.
