Trinity Health Livonia is a beautiful full-service, 304-bed acute care hospital, located in Livonia, MI, that provides comprehensive care, including a 24-hour emergency department, general medicine, inpatient and outpatient surgery, physical medicine and rehabilitation, intensive care unit, cancer, cardiology, geriatrics and birthing and women's health. POSITION PURPOSE
Provides continuous observation and surveillance of assigned patients. Summons the nursing staff if the patient requires assistance, control or other nursing intervention. Knowledgeable and supportive of patients, the confidentiality of HIPAA protected information, and other related hospital policies.
ESSENTIAL FUNCTIONS MAY INCLUDE:
Complete hand off at shift change using RN to Patient Companion Report sheet.
Put name on white board.
Use proper hand washing technique.
Use proper phone etiquette.
Understand and follow four types of Infection Control Precautions (respiratory, droplet, contact, C-diff).
Use surface anti-contaminant process correctly.
Replace nasal cannula oxygen.
Follow suicide protection policy.
Contact RN with signs of pain or change in patient condition.
Encourage incentive spirometer use.
Assist with SCD placement/removal.
Use basic safe transfer techniques.
Ambulate patients with RN approval and direction.
Use chair alarms/bed alarms.
Maintain safe environment for patient in soft/waist restraints.
- PCs may sit with patients in 4 pt. restraints in Livonia and Chelsea.
Feed patients who do not have precautions or restrictions.
Follow "Fall Risk" precautions.
Assist RN and assistive staff as directed within scope of job description including ADLs (feeding, oral care, bathing, and toileting).
Update RN on regular basis with patient condition report.
Give completed Patient Monitoring Log to RN during midnight shift or upon patient discharge. Obtain PLUE label for log before submitting to RN.
Assist RN or PCT with use of lift equipment.
Read signs outside of patient rooms for NPO/Fall Risk/Telemetry/isolation notifications
Keep patients room free of clutter and tidy.
Livonia-use of the activity cart with confused patients.
Accompany patient to testing unless directed otherwise by RN.
MINIMUM QUALIFICATIONS
Education
- High School diploma.
Licensure
- None.
Training and Experience
Ability to maintain professionalism and self-control in difficult or stressful conditions.
Ability to maintain attention for long periods.
Job Knowledge
- On the job training.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
Physical Demands:
Capable of observing patients and remaining alert at all times.
Must be able to communicate effectively with hospital staff. Required to respond to patient behavioral changes by reporting and requiring assistance.
Special Demands:
- Ability to sit for long periods of time and remain alert without distraction from personal business or other personal diversions.
