Primary Territory:

This position primarily covers Berwyn, Broadview, Riverside, and Chicago , with exact territories provided by the manager for each role.

Job Summary:

As a Medical Social Worker, you’ll provide professional social services to clients based on physician orders, in compliance with state and federal regulations, and according to agency policies and procedures. You will play a key role in delivering compassionate, patient-centered care to those in need.

Key Responsibilities:

  • Deliver high-quality social services to patients in their homes

  • Collaborate with the healthcare team to ensure continuity of care

  • Support patients and families through the challenges of illness and treatment

Qualifications:

  • Master’s Degree in Social Work from an accredited institution

  • Minimum one year of experience in a healthcare setting

  • Current Social Worker license in the state of Illinois (or applicable registration)

  • Strong commitment to ongoing professional development

  • Adherence to agency policies, ethical standards, and core competencies

  • Valid driver’s license and reliable transportation

  • Home care or hospice experience preferred
