Job Overview:

The Geriatric Advanced Practice Nurse is a vital member of the care team who works collaboratively with our geriatric medical team on the management and oversight of the patients of the Geriatric Health Center.

Qualifications:

Required:

  • Certified Nurse Practitioner in the state of New Jersey,

  • Geriatric specialty or subspecialty preferred,

  • Master s degree in Nursing from an accredited program

  • BLS Certified

Essential Functions:

  • The APN performs comprehensive health assessments and management of patients with acute and/or chronic health problems,This may include obtaining patient histories and performing physical examinations and writing orders and daily progress notes as indicated,The APN will be expected to assess and manage common health problems as well as medical problems specific to the elderly population,The APN will order and evaluate pertinent diagnostic tests including but not limited to: Radiology, laboratories and ECGs,

  • They will order pharmacological agents and non-pharmacological treatments for appropriate medical conditions,

  • Education of patients and/or caregivers regarding processes, medical intervention and health promotion are included in daily care,

  • The APN will be responsible for establishing home visit care for patients within a 12-mile radius,

  • The APN will refer and communicate pertinent information to the physicians and care team providers as appropriate,

  • They will serve as a resource for clinical and ancillary staff in regard to Geriatric patient and disease management,

  • The APN will address any clinical patient care issues, concerns and follow up as needed,

  • The APN will also be a resource to and support other geriatric programs at the institution as necessary,

  • Provides a compassionate and supportive environment with prompt and consistent service

  • Adheres to patient rights: provides for the confidential treatment of all communications and records,

  • Demonstrates professionalism with families, visitors, physicians, co-workers and manager,

  • Listens and communicates effectively,

  • Organized work sets priorities with a minimum of supervision and seeks guidance as appropriate.
