Job Summary

Medication reconciliation technician (MRT) is responsible for accurately collecting and entering patient’s current medications, allergies and medication sensitivities into the hospital’s information system upon admission. MRT will primarily be working in the Emergency Room with frequent contact with patients, nurses and physicians. This position requires the technician to be flexible with scheduling needs and be able to perform regular technician duties including IV preparations, medication distribution, Pyxis maintenance, assistance with inventory management, clerical duties, and other responsibilities as required by a pharmacy manager.

Essential Duties and Responsibilities

• Reconcile patient’s home medication list accurately by verbal communication with patient, review of Centricity records, review of external medication history, communication with retail pharmacy and other facilities as appropriate.

• Navigate effectively through Hospital’s information system. (EMR, Centricity)

• Communicate respectfully and effectively with Providers and Nursing.

• Problem solving skills with Nursing and Pharmacists

• Perform the duties of Pharmacy Technician in respect to IV preparation, medication distribution, Pyxis management (prepare medications for restocking of Pyxis medication stations), and other duties as required.

• Supports and promotes the mission and values of Covenant Health Ministry.

• Other duties as consistent with this role.

• Rotating schedule 1st & 2nd shift

Education and Experience

• High School Diploma or GED or Technical school

• Active Maine Pharmacy Technician License, and Medical Reconciliation Technician(MRT) Certification

• 3 years’ experience in a healthcare related position with 5 or more years’ experience as apharmacy technician preferred.

• Experience with Pyxis Med Stations and Meditech or other pharmacy software preferred.

An equivalent combination of education and experience which provides proficiency in the areas of

responsibility listed above may be substituted for the above education and experience requirements.

Working Conditions/Physical Demands

Requires color discrimination and accurate depth perception. Fine motor skills are required including grip strength and pinch grip. Must possess the physical and mental abilities to perform the tasks normally associated with this position that involve a combination of sitting, standing, walking, bending, stooping, twisting, and reaching. Lifting/carrying frequently up to 35 pounds, and occasionally up to 50 pounds. Some stress is related to a high level of responsibility for quality care. The ability to speak, read, write, and understand written instructions in English is required. Working and preparation of chemotherapeutic drugs.
