Duties include assessing, planning, | | implementing, monitoring, and evaluating client behaviors and | | outcomes while ensuring appropriate level/type of service is | | provided.  | +-----------------------------------------------------------------------+ | Position Description: | | | | - With close supervision; conducts interviews and administers | | assessments and screening instruments as defined by certification | | - Maintain caseload and productivity by ensuring regular contact | | with clients to monitor and assess progress | | - Lead educational groups and/or facilitate group curriculum | | - Write service plans, review progress and determine updates needed | | in collaboration with client and team | | - Utilize approved best practice model working with clients in | | individual or group setting | | - Deliver on-going services to clients which may include: crisis | | intervention, advocacy, medication monitoring, and referral and | | linkage to community resources as needed | | - Ensure compliance with applicable standards and agency policies | | - Document all clinical activities to meet specific program, funder | | and regulatory criteria timely and with accuracy | | - Participate in team meetings and assist in making recommendations | | for client status changes | | - Adhere to professional standards as outlined by protocols, rules | | and regulations | | - Other duties as assigned | | | | +------------------------------------------------------------------+ | | | Required Knowledge, Skills and Abilities: | | | | | | | | - Ability to provide group and individual substance use | | | | disorder services | | | | - Excellent organizational and time management skills | | | | - Effective verbal and written communication skills | | | | - Demonstrate strong ethical and professional behavior.   | | | | - Ability to relate to and interact with a wide variety of | | | | people in an ethical, culturally sensitive and respectful | | | | manner | | | | - Strong problem solving skills and ability to multi-task | | | | while prioritizing | | | | - Ability to utilize technology including electronic service | | | | documentation system | | | +------------------------------------------------------------------+ | | | Job Requirements: | | | | | | | | - Associates Degree in Behavioral Science or related field or; | | | |   | | | | - Equivalent work experience as allowed/required by regulatory | | | | bodies | | | | - CDCA  | | | +------------------------------------------------------------------+ | | | Essential Functions/Physical Demands: | | | | | | | | - Positional: Driving in accordance with job duties assigned. | | | | Frequent standing and walking.
