Graduate from an accredited school of nursing

Current New Hampshire license for a Registered Professional Nurse.

Two years experience in applicable area.

Ambulatory care and/or triage experience preferred.

Current BCLS certification required within 30 days of hire.

PART TWO:FUNCTIONAL RESPONSIBILITY

Position Objective
*
Responsible for case management of clinic patients. Acts as resource for providers, patients and support staff regarding clinical and administrative support decisions. Works closely with Clinical Nursing Supervisor to assure efficient systems are in place to insure high quality patient care. The registered Professional Nurse is accountable to meet the nursing needs of a defined group of patients utilizing the medical plan of care in accordance with the New Hampshire Nurse Practice Act and the Patient Bill of Rights.

Knowledge of medications and their correct administration based on age of the patient and their clinical condition.

Follows the seven medication rights and reduces the potential for medication errors.
*
Performs patient care using an integrated approach that optimizes patient safety and reduces the likelihood of medical/healthcare errors.

Ability to formulate an individual plan of care, revise plan as indicated by the patient’s response to treatment and evaluate overall plan for effectiveness.
*
Communicates clinical information appropriately to provide for an interdisciplinary approach to the patient’s needs. Collaborates with all ancillary departments.
*
Coordinates and supervises patient care, as necessary.
*
Demonstrates an ability to organize and prioritize job function.
*
Demonstrates critical thinking skills, using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

Performs physical/psychosocial assessment to collect data: follows the written plan of care, standards, practice guidelines.
*
Utilizes assessment data to identify patient care needs, to include patient education and discharge planning.
*
Recognizes and reports abnormal assessment data and incorporates findings into the plan of care: validates and revises expected outcomes based on assessment.
- Accurately and legibly documents all patient care activities, including but not limited to, wound assessment, patient care teaching, demonstrated progress toward goals, medication administration, nursing flow sheets and all assessment forms.
