Works under the supervision of the Director of Resource Management. Responsible for assuring timely/safe discharge planning and that patient care is provided in a timely, cost-effective manner.

Qualifications include BSW; MSW preferred; two years experience in a health care environment; previous experience in hospital discharge planning preferred; ability to read and understand the requirements of the North Carolina Peer Review Organization and Medicare Conditions of Participation; familiarity with Microsoft Office; excellent communication and organizational skills; diplomacy, tact, and a positive approach to problem solving.

Days and hours vary to include weekends and holidays.
