The Case Manager will provide direct case management services to homeless and at-risk veterans and their families. They will screen veteran's eligibility for the program and facilitate the intake process. Case Managers will coordinate care with VA personnel, assist veterans in identifying education and training goals, and identify mainstream needs and facilitate the process to meet those needs.

QUALIFICATIONS

  • Bachelor’s degree in a Human Services related discipline preferred.

  • Related employment experience may be substituted for degree requirements on a 2:1 ratio

  • Experience working with homeless and/or at-risk veterans and families preferred.

  • Ability to work independently and maintain strict confidentiality.

  • Strong verbal and written communication skills.

  • Proficiency in Microsoft Office Software (Word, Excel, etc.)

  • Ability to communicate and work effectively with a diverse group of residents, staff and community partners.

  • Must have a valid Driver License with good motor vehicle record.

  • Must be able to pass a criminal background check.

  • Veteran Status Preferred

Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to focus. The employee must have the ability to operate a telephone, desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
