(Employees in this position will perform the following general tasks and duties in working in various clinics. Not all clinics will perform the same duties or tasks, and employees will have specific tasks assigned as directly associated with their clinic assignment.)
Under the supervision of the Operations Manager, provides comprehensive nursing services including assessment, education, counseling, and prevention services to individuals and families to promote health throughout the community. Participates in the operation of health centers and programs, maintains patient and program activity records, and educates patients on a variety of health topics.
Essential Functions
Interviews patients and conducts health assessments including conducting physical assessments, obtaining blood pressure, temperature, measurements, and other data and developing information on health and diet history and family health status. Provide drive-thru services for testing and/or immunizations.
Assesses various tests and screening results and correlates them with health history and physical assessment. Determines absence or presence of abnormalities in the health history, physical examination, and health maintenance activities. Reviews lab results and specialist reports and refers to the appropriate medical provider in a timely manner.
Arrange diagnostic testing, specialized medical procedures, and specialty referrals. Completes necessary paperwork, contacts specialist's offices, hospitals, labs, or other facilities to schedule appointments. Completes paper work, mails copies to patient and/or caregiver, and refers to proper personnel to complete referral authorizations as needed. Assist with specialized medical procedures.
Obtains appropriate authorizations for medical care, communicating with patients and case managers regarding issues related to authorizations, benefits, and medical treatment, and surgery schedules. Resolves problems with insurance providers. Complete prior authorization (PA) requests.
Refers patients for diagnostic tests. Coordinates paperwork including prior authorizations obtained. Notifies patient of appointments and educates regarding required preparation.
Provides Case Management/Care Coordination evaluation to patients who have high risk health care needs to ensure optimal outcome of care or refers them to other department staff for evaluation. Perform Care Management between the health center and hospitals, emergency rooms, and other outside facilities.
Reviews written prescriptions with patient and/or caregiver, providing education regarding side effects, dosage, and any special instructions.
Assists medical provider with physical assessments and procedures as needed. Perform treatments, administer medications and injections and performs testing per provider orders. Educate patient and/or caregiver in the proper way to collect specimen in order to obtain accurate results.
Maintains patient charts, ensuring proper documentation of clinic visits, diagnosis, treatments, and medications. Document patient's diagnosis and procedures for accurate billing.
Triages calls or visits from patients and/or caregivers. Determines needs and initiates appropriate action such as intervention, appointment, after hour clinic referral, or answering patient questions. May initiate appropriate action by using standing orders for over-the-counter medications or referring to provider, calling parents/guardians, and/or administering medical provider ordered medications.
