Job Title: Patient Navigator - Peer Support Specialist

1.0 FTE, Full Time, 40 hours per week. 2nd shift. Flexible schedule to be discussed at interview. Hours of operation are Mon-Fri 11a-8p, Saturday/Sunday 9a-2p. Every other weekend rotation. No holiday or on-call.

POSITION SUMMARY:

The patient navigator is a certified peer support specialist (CPSS) who works in the Crisis Walk In Center; the navigator helps guide the patient through their experience seeking help for behavioral health conditions. The navigator collaborates with the care team, including nurses, counselors and psychiatric providers to assure that the patient has a follow up care plan. The navigator is an advocate and helps the patient navigate the healthcare system and addresses social determinants of health and barriers to care.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Meets with patients presenting to the Crisis Walk In Center, engages in recovery oriented dialogue and supports the patient during their visit.

  • Collaborates with care team members to address the presenting needs of the patient.

  • Facilitates communication between the patient, family members, community supports and health care providers.

  • Completes assessment of social determinants of health and in conjunction with the team, develops a plan to address them.

  • Ensures continuity of care and linkage to community resources based on needs assessments.

  • Schedules patients for follow up care as directed by the treatment team.

  • Documents interventions and time spent with the patient in the electronic medical record.

  • Makes outreach follow up call to patients served in the Crisis Walk In Center.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Maintains knowledge of registration and scheduling to support patient flow and for follow up care.

  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • High school diploma or GED

  • Current certification as a Peer Support Specialist is required.

  • Three (3) years of customer service or administrative support experience.

PREFERRED QUALIFICATIONS:

  • Experience working in a health care or behavioral health care setting.

  • Experience working successfully on a multidisciplinary team.

The individual must have the ability to apply principles of reasoning and problem solving to resolve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
