Position Summary:

Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

Reports to: Director of Care Management

FLSA: Exempt

Minimum Qualifications:

Minimum Education Bachelor’s degree in Social Work required. Master's degree in Social Work preferred Drag Edit Delete

Required Skills Experience in Acute Care is preferred

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervisor. Drag Edit Delete

Required Skills Certifications: Basic Life Support (BCLS) preferred Drag Edit Delete

Required Licenses [Arizona, United States] Social Worker Licensed Clinical Social Worker in Arizona preferred
