Position Summary:

Responsibility for the daily operations of Peninsula’s Behavioral Health Home including; oversight of utilization of the “Care Coordination Tool” to prevent gaps in the care of TN Health Link (THL) members, liaison with primary care providers to assure high quality coordination of care, oversight of the development of comprehensive person-centered care plans for Health Link Members. Responsible for direct or collaborative supervision of all staff providing services to TN Health Link members. Assists in the development, management and implementation of comprehensive strategic initiatives in the areas of clinical effectiveness, quality improvement, regulatory compliance and related areas to support and enhance achievement of organization’s goals. Plans and budgets for the department and develops and implements goals and objectives as necessary to achieve outcomes defined by THL.

Responsibilities

  • Accountable for oversight of THL service delivery through clinical supervision, scheduling and evaluation of staff, training, direct observation, consumer feedback, consultation with physicians, role modeling, and providing other resources to staff as needed.

  • Assures the development of an effective “comprehensive care management plan” for each THL member

  • Manages and tracks data regarding Health link member’s health status and assures that each member receives appropriate services as delineated in the “comprehensive care management plan

  • Responsible for developing and executing successful strategies to meet THL designated outcome and efficiency measures

  • Facilitates development of collaborative relationships and agreements with primary care physicians and other external medical providers as needed to meet the healthcare needs of Health Link members

  • Responsible for quality and effectiveness of ongoing communications with primary care physicians and other external medical providers to assure successful care coordination

  • Responsible for hiring, training, orientating and evaluating performance/competency, and discipline of designated employees. Collaborates with the Director of Clinical Services for position management, staffing and scheduling

  • Ensures Health Link compliance with regulatory agency requirements (The Joint Commission, state agencies, etc.), as related to patient care and clinical documentation activities

  • Monitors patient complaints and provides recovery/conflict resolution for customer or patient activities as needed

  • Participates in the development, implementation, and monitoring of annual operating budget

  • Performs direct care services as needed

  • Is a member of the Peninsula Outpatient Leadership Team and as such participates in policy development, strategic planning and coordination of services between the hospitals and clinics

  • Serve as liaison with managers, clinical services, and other departments responsible for coordinated patient care along the continuum of care

  • Performs other related services as assigned or requested

Qualifications

Minimum Education:

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

Minimum Experience:

A minimum of two years’ experience in behavioral health preferred. Management experience and previous experience in population health management is preferred.

Licensure Requirement:

Current licensure as a registered professional nurse in the State of Tennessee. Current CPR certification required.
