As a Community Support Specialist, you'll work with clients in various capacities, providing mentorship, guidance, and hope to help them overcome challenges and achieve their goals. You'll have the chance to work in different settings, including clients' homes, clinics, and other community locations, ensuring that each individual receives personalized support tailored to their unique needs. Key Responsibilities:

  • Holistic Care & Collaboration: Focus on personal strengths and collaborate with a team to define goals and assess progress. Develop tailored treatment plans targeting symptoms, fostering independence, and enhancing skills. Co-create documentation and provide regular updates to share objectives.

  • Skills Development & Functionality: Identify skill gaps and implement adaptive interventions for diverse skills. Create solutions for environmental challenges linked to mental health or substance issues.

  • Positive Outcomes & Proactive Communication: Address multiple facets of clients' lives, such as employment, housing, and relationships. Ensure continuity of care, managing missed appointments and handling crises effectively.

  • Knowledge Sharing & Education: Share insights on mental health conditions, substance use, symptom awareness, etc. Promote medication adherence, wellness planning, coping mechanisms, and building social abilities.

  • Preventing Relapse & Enabling Independence: Identify risks and develop robust plans to prevent potential relapses. Foster accessible services and connect clients to vital programs.

  • Crisis Response & Thorough Documentation: Respond swiftly to mental health emergencies using discernment and expertise. Maintain detailed records, capturing essential details and tracking follow-ups.

  • Engaging Staff Contribution: Participate in team meetings, training sessions, and workshops to enhance proficiency. Manage caseload effectively, partnering with supervisors.

  • Community-Based Solutions & Behavioral Techniques: Extend quality, empathetic services beyond clinical boundaries. Use efficient methodologies such as motivational interviewing, shaping, and cognitive processes.

  • Ongoing Learning & Commitment To Excellence: Seize learning opportunities, attend conferences, and advance competencies. Commit to delivering nurturing recovery and resilience support for every client.

  • Bachelor's degree in one of the following fields of study: Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation Counseling, Education (Teachers!)OR...

  • Associate of Applied Science in Behavioral Health Support degree from an approved institutionOR....

  • Work experience in a related human services field may substitute for educational requirements. *Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required.OR....

  • Qualified Addictions Professional (QAP) CADC, CRADC, CRAADC, CCJP, RADC-P, RADC, CCDP, CCDP-D
