The HCBS Wait List Care Coordinator is responsible for establishing a set of person-centered goal-oriented, culturally relevant, and logical steps to ensure that a person receives services in a supportive, effective, efficient, timely and cost-effective manner. Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into community.
Working Schedule: Schedule is Monday through Friday between the hours of 8 am to 5 pm. This position is a field-based position with a home-based office. You will work from home when not in the field.
Location: Shawnee county and surrounding areas
Local travel up to 75% and mileage is reimbursed at current government rate.
Primary Responsibilities:
Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care
Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
Ensure the care plan/plan of service includes covered services, EPSDT, in lieu of services, value-added benefit, informal supports, and addresses social determinants of health
Communicate with all stakeholders the required health - related information to ensure quality coordinated care and services are provided expeditiously to all persons
Advocate for persons and families as needed to ensure the persons needs and choices are fully represented and supported by the health care team
Conduct home visits in coordination with person and care team, which may include a community service coordinator
Conduct in-person visits which may include nursing homes, assisted living, hospital, or home
Serve as a resource for community care coordinator, if applicable

Required Qualifications:
Bachelor’s Degree (or higher) in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area
1+ years of experience working with persons with long-term care needs
1+ years of experience working within the community health setting in a health care role
Intermediate level of computer proficiency including the use of Microsoft Outlook, Teams, and multiple web applications
Ability to travel 75% of time for role
Valid driver's license, current automobile insurance and access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Preferred Qualifications:
Licensed Social Worker or clinical degree
At least one year experience working with persons receiving services on one of the home and community based waivers in KS
Background in managing populations with complex medical or behavioral needs
Experience with electronic charting
Experience with arranging community resources
Soft Skills:
Strong communication and customer service skills both in person and via phone
Ability to work independently and maintain good judgment and accountability
Demonstrated ability to work well with others
Strong organizational and time management skills
Ability to multi-task and prioritize tasks to meet all deadlines
Ability to work well under pressure in a fast-paced environment
Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
