FULL-TIME [Nonexempt] : 40 hours per week

Coordinated Care Services Inc (CCSI) (https://positions.paylocity.com/PositionManagement/wizard/2033530) seeks a talentedAdult Single Point of Access Program Associateto contribute their expertise in providing essential business services to our provider organizations and customers. TheAdult Single Point of Access (A-SPOA) Program Associateis responsible for entering, triaging, and routing referrals for A-SPOA. This includes communication with partnering agencies, coordination and facilitation of complex case review sessions and partnering with existing Department staff to build the A-SPOA committee in alignment with building expertise in the Broome County adult system of care.

CCSI is partnering withBroome County Mental Health Departmentto hireanAdult Single Point of Access Program Associate.This position is employed by CCSI and working on behalf ofBroome County.

In this role, you will

  • Receive, process, and monitor referrals for services via A-SPOA.

  • Attend, participate and assist with the facilitation of A-SPOA Committee meetings.

  • Documentation and tracking of case statuses and progress while under the supervision of A-SPOA.

  • Develop and manage protocols for access to, and oversight of, high-level mental health service process(es).

  • Develop, implement, and maintain system for tracking referrals, service slots, availability, and utilization.

  • Design and implement monitoring protocol to ensure appropriate utilization management and utilization review procedures are in place.

  • Respond to informational inquiries regarding the referral process and basic aspects of BCMHD functions.

  • Maintain appropriate documentation for all projects and job tasks as assigned.

  • Compile and submit performance reports to BCMHD leadership and Broome County Stakeholders as requested.

  • Provide program vision and development

  • Support intra- and interdepartmental initiatives and planning functions within Broome County Mental Health Department.

  • Travel as necessary to fulfill assigned responsibilities

  • Other duties as assigned.

Requirements

  • Bachelor’s degree in Human Services or related field with minimum three (3) years of relevant work experience with adults with Serious Mental Illness (SMI), families with children with Serious Emotional Disturbance (SED), individuals with Co-occurring Disorders, or individuals with Intellectual and Developmental Disabilities (I/DD),OR

  • Equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) ).

  • Preferred: current, valid Licensure by the New York State Education Department as a:

  • Qualified Mental Health Staff Personin accordance with 14 NYCRR 595;OR

  • Non-Physician Licensed Behavioral Health Practitioner(NP-LBHP) in accordance with 14 NYCRR 598 or 599;OR

  • Licensed Practitioner of the Healing Arts(LPHA) in accordance with 14 NYCRR 512.4
