JOB REQUIREMENTS: JOB SUMMARY The Federal Case Manager works directly under the Assistant Residential Services Director. The primary emphasis of this position is on developing and coordinating reentry programming that ensures continuity of care for residents. The Federal Case Manager will meet with residents, maintain collateral contacts with contractors and community resources, provide documentation of resident activities, attend required meetings/trainings, and reporting. The Federal Case Manager will ensure the requirements of the Federal Bureau of Prisons Statement of Work (SOW) and the Residential Services Department Policies and Procedures are met. The Federal Case Manager must consistently present a solid profile of the organization to the contractors and the community. ESSENTIAL FUNCTIONS 1. Individualized Program Plan: Within 10 calendar days of entry, the Federal Case Manager will create with each resident, in consultation with the Social Service Coordinator and Employment Placement Specialist, an Individualized Program Plan to address the residents needs and risks concentrating on, but not limited, to the following areas: housing, employment, financial management, physical health, and education. The Federal Case Manager will work side by side with residents to ensure all tasks are completed from start to finish. Housing Assist residents in search for stable, secure, cost-effective housing. Ensure residents understand and complete housing applications as deemed necessary. Employment Assist the Employment Placement Specialist as needed in employment. Ensure legal status is indicated to the employer prior to the resident beginning employment and is documented in the resident\'s file. Ensure on-site visit is completed within the first 7 calendar days of employment and that the visit is documented in the resident\'s file. Ensure at least monthly a phone or on-site visit is conducted at the resident\'s place of employment to substantiate attendance and discuss any concerns that may have arisen and ensure these are also documented in the resident\'s file. Financial Management Assist residents in establishing and maintaining sound financial practices and monitor all monetary transactions of residents. Assist residents in creating/maintaining a budget. Physical Health Assist residents in obtaining a resolution for any physical health problems. Ensure communication regarding all mental health appointments (including aftercare) is relayed to residents\' individual Therapist. Education Assist residents in locating resources for obtaining GED, HSED or continuing education. The Individualized Program Plan will clearly identify how RRC staff will prioritize and assist the resident in addressing all elements of the plan, to include specific program activities and a timetable for achievement of these goals. 2....
