SMP Health - Ave Maria Licensed Social Worker or Social Services Designee in Jamestown, North Dakota

Full-time or Part-time position to work Monday through Friday.

This is an excellent opportunity for a compassionate individual to join the loving care team at our faith-based skilled nursing and rehab facility. We are looking for a LSW (or Social Services Designee) who enjoys teamwork, loves developing relationships, and thrives on making a difference in people's lives every day. Long-term care experience helpful but not required. Salary dependent upon experience and excellent benefit package.

* ESSENTIAL FUNCTIONS FOR LICENSED SOCIAL WORKER: *

  1. Provides initial contact of residents moving to the facility on their assigned neighborhood. Orientation to the surroundings, discussion of facility policy and support in adjusting to the changes of the resident's life are included in working with the admission process. Includes discharge planning as part of the continuum of care starting with the admission process.
  2. The Social Worker utilizes a resident focused approach This position provides social work services to residents and families in such areas as interpersonal relationships, stressful life events, disabling diseases, terminal illnesses and adjustment to both living in a nursing home as well as the aging process. The Social Worker is able to lead groups and provide supportive visits for residents and/or families. The Social Worker can provide educational programs for residents, families, staff and community organizations.
  3. This position participates in resident care planning to develop and assure that department services meet each resident's needs. This position is responsible to encourage resident/family participation and input in the care planning process.
  4. This position participates in discharge planning which includes transportation arrangements, arrangement for transfer to other facilities, home evaluations, and assistance for residents and families to find outside services for independent living.
  5. The Social Worker prepares reports such as social histories, progress notes in resident records, discharge notes, and notes in resident care plan meetings. This position maintains reports and statistics on assigned residents and prepares letters and reports for outside agencies on referrals and for state agencies as needed. The Social Worker performs computer data entry duties to complete resident assessments and other reports/records. This position performs duties such as photocopying and maintaining Social Service files.
  6. Performs safety and infection control procedures to insure a safe and clean environment. Attends mandated meetings for the well being of the facility. This includes understanding and following the procedures relating to wandering residents, fire, severe weather, universal precautions and safe operation of any equipment used.
