Certified Nurse Aide , Thome PACE Day Center & In-Home

FULL TIME 30-40 HOURS/WEEK

Salary Range:$17.85 To $19.89 Hourly

*Hours: *40 hours per week | 8a -- 4:30pm

Overview:

The Certified Nurse Aide (CNA) performs patient care activities and other tasks to assist the primary care, therapy, and adult day health team members.  

Thome PACE® serves Jackson, Lenawee, and Hillsdale counties providing participants with a wide range of medical and supportive services in our Day Center located in Jackson, Michigan or in the comfort of their own homes. 

What you'll do:

• Assists in providing and/or maintaining optimal physical and emotional comforts to participants.
• Assists participants toward achieving maximum self-reliance and independence.
• Communicates with participants, families, staff, and the staff of other agencies.
• Participates in interdisciplinary team meetings.
• Provides basic ADL nursing care.
• Documents treatments/procedures, observations, nursing care, and participant activities.
• Reports changes in the participant's condition to the pertinent interdisciplinary team member.
• Demonstrates active participation in QI processes.
• Participates in the process of teaching and learning with others.

What it Takes:

• Certified Nurse Aide (CNA) valid in the State of Michigan required.
• CPR certification required.
• Must have reliable transportation-with valid driver's license willingness to drive to Jackson, Lenawee and Hillsdale Counites.
• Flexibility to provide coverage in the evenings and weekends
• Minimum 1 year of experience working with elderly is required.
