Henry Ford Health System Coordinator - Program Performance | Clinically Integrated Network | Jackson, MI in Jackson, Michigan

GENERAL SUMMARY:

This position is responsible for coordination of high-impact network programs and development of network-level performance reports for committees, payor applications, progress reports and governmental programs.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Coordinate timely completion of required activities and reports to maintain compliance with payor and governmental programs including Comprehensive Primary Care Plus, State Innovation Model, PGIP, and other key initiatives.

  • Work with Business Intelligence, Health Information Technology, consultants, practice representatives and data specialists on development of new data management and reporting tools including design of clinical program metric reports, scorecards and other routine and ad hoc reports as required for network operations, payor contracts, and population management programs.

  • Gather and interpret multiple sources of data and information, generate summaries, and complete and submit applications, progress reports, surveys and budget updates as needed to meet program requirements.

  • Maintain program budget and tracking expenditures/transactions as needed.

  • Ensure practices have accurate information, tools, and support to maintain accountability for participation requirements and successfully contribute program and payor data and other progress information.

  • Train and educate staff, community partners and physician practices on key elements of program requirements, practice obligations and resources, data collection tools and procedures, quality standards and measurement methodologies, and interpretation of findings and reports.

  • Manage communications through media relations including leading development of the network’s Annual Value Report summarizing key outcomes and impact of network activities.

  • Other duties as assigned.

EDUCATION/EXPERIENCE REQUIRED:

  • Master's degree in business administration or relevant field, or the equivalent combination of education and/or experience.

  • Three (3) years of healthcare experience.

  • Previous project management training or completion within six (6) months of hire.

  • Knowledge of program management and development procedures.

  • Knowledge of budgeting, bookkeeping, and reporting.

  • Excellent time-management and organizational skills.

  • Outstanding professional, appropriate, effective verbal and written communication skills.

  • Detail-oriented and efficient.

  • Expert competency in Microsoft Office is required (i.e.: Word, Excel, Access, and Outlook).

  • Expert competency in data collection, management and reporting systems including database design and use.

  • Demonstrated ability to perform independent reporting on key organizational metrics.

  • Demonstrated understanding of privacy laws and standards.
