Community Health Network has an excellent opportunity available at Community Heart and Vascular Cardiac Clinic. The Ambulatory Triage LPN is responsible for addressing patient's condition/symptoms virtually, giving appropriate clinical direction, education, and recommendation(s) for disposition. The Ambulatory Triage LPN for the Cardiac Clinic is a hybrid remote/work from home position; 50% working on-site and 50% working remotely.

Exceptional Skills and Qualifications

  • Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience. (Required)

  • Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency (IPLA). (Required)

  • One to two years of ambulatory and/or triage experience in a clinical setting (Preferred)

  • Possesses knowledge of necessary computer systems including Microsoft Office, Outlook, and network electronic medical records (EMR).

  • Supports the mission, vision and PRIIDE values of Community Health Network.

  • Utilizes critical thinking skills, education, mentoring, guidance, and interventions to achieve goals of care virtually.
