Pay & Benefits of the Case Manager:
$50,000 to $55,000 salary per year.

Responsibilities of the Case Manager :
Conduct thorough assessments of clients' strengths, needs, and challenges to gain a comprehensive understanding of their unique situations.
Collaborate with clients to develop individualized action plans that address their specific goals and aspirations.
Determine program eligibility and enrollment.
Identify and connect clients with appropriate resources, services, and community-based programs to meet their physical, emotional, and social needs.
Facilitate referrals to external agencies and service providers, ensuring seamless access to required assistance.
Provide up-to-date information on current available employment and training opportunities and application requirements.
Assist in identifying potential participants who meet the eligibility criteria for the grant programs through various means, including database searches, community networking, and referrals.
Provide one-on-one assistance to potential participants throughout the application process, guiding them through the application requirements and submission.
Requirements of the Case Manager :
High school diploma or equivalent (associate’s degree or relevant certifications are a plus).
Proven experience in participant recruitment, community outreach, or similar roles, preferably within grant-funded programs.
Excellent communication and interpersonal skills to engage with diverse audiences and build rapport.
Demonstrated ability to develop and execute successful outreach strategies.
Knowledge of digital marketing tools and social media platforms for effective outreach
Strong organizational and time management skills to manage multiple outreach activities effectively.
Empathy and sensitivity in working with diverse populations and understanding their needs and challenges.
