Location: 100% Remote (Supporting Pacific Time Zone; CANDIDATES MUST BE FROM NV, OR, AZ, TX
Schedule: Must be available for rotating weekends and holidays
Job Summary:
We are seeking a highly organized and compassionate medical professional to join our team. This role will support high-volume fax and voicemail management, assist with data entry, and work with the Care Coordination team to facilitate the discharge process and ensure a smooth transition for ambulatory patients. The ideal candidate will have prior experience with Epic EHR and be open to weekend shifts.
Key Responsibilities:
Collaborate with Case Managers and Social Workers to assess and execute the Transition of Care (TOC) and Discharge (DC) plan for patients.
Develop, monitor, and update the TOC plan, ensuring timely discharge by addressing barriers and coordinating with the medical facility and post-acute providers.
Assist in obtaining post-discharge referrals, authorizations, and coordinating transportation services when necessary.
Provide clerical support including faxing, scanning, data entry, and document preparation.
Communicate with patients, families, and external vendors to ensure the transition is smooth, timely, and effective.
Participate in daily team huddles, CC department meetings, and actively contribute to resolving barriers in the care process.
Maintain accurate documentation within the electronic medical record (EMR), including patient preferences and referral status.
Deliver the Medicare “Important Message” (IM) to patients and inform them of their rights.
Build and maintain positive relationships with patients, families, and healthcare providers.
Comply with HIPAA regulations and confidentiality standards when transmitting patient information.
Qualifications:
Required:
High School Diploma or equivalent (or equivalent experience).
1+ year of recent, relevant experience in healthcare, preferably in a care coordination or similar role.
Knowledge and experience with Epic EHR.
Strong oral and written communication skills.
Ability to work in a fast-paced environment with competing priorities.
Intermediate proficiency in Microsoft Office and data entry.
Strong interpersonal, organizational, and time management skills.
- Preferred:
Experience in working with discharge planning, care coordination, or hospital settings.
Familiarity with the transition of care process and knowledge of post-acute services.
Ability to perform clerical tasks including faxing, scanning, and data entry.
Must be able to handle physical demands associated with a fast-paced office environment.
