Empower Lives - Join Us as a Care Transition Nurse!

At Post-Health Home Care, we're on a mission to uplift communities through compassionate care. As a Care Transition Nurse (CTN), make a lasting impact by coordinating quality home-based services and guiding patients to the best care.

Collaborate with physicians, hospitals, and care facilities to provide coordinated and quality home-based services. Be the beacon of support and guidance in our healthcare landscape.

Assist discharge coordinators in coordinating home care services, empowering patients to make informed choices for their wellbeing.

Provide patients and families with compassionate education on home care regulations and services.

Qualifications

  • LPN or RN experience is required

  • Care Transition expertise and Sales experience

  • Home Health experience is highly preferred

Pay Range

$27.45 - $39.80 /hour
