Personal Care Provider

JOB SUMMARY This position is responsible for providing assistance in the home, errands, and unskilled personal care services to clients in the home setting.

ESSENTIAL JOB FUNCTIONS Maintains confidential data and information according to HIPAA protocols. Assists with quarterly audits of chart. Communicates to Program Coordinator and/or Administrator areas of concern related to client/caregiver interaction, client environment, needs, changes, or client safety and reports such information as required by agency policy and State regulations. Actively participates and makes recommendations on the quality improvement programs. Follows infection control standards. Prioritizes tasks in order of importance for safety of client. Performs housekeeping tasks, according to the care plan, as needed for health and comfort in the client's home, including making and changing bed linens, cleaning bathroom, personal laundry, cleaning kitchen area (including cleaning cupboards, refrigerator, stove, counter tops, floors), trash removal, dusting, vacuuming, grocery shopping, errands, and food preparation. Assists with unskilled personal care as assigned by the plan of care., including stand-by assist with bathing, toileting; hair, skin, foot care, and dressing. Meets clients' needs, within reason, consistent with the goals of the program. Advises program coordinator or supervisor of conditions or changes of any non-emergency medical situations. Initiates Emergency Response System when necessary. Advises program coordinator or supervisor of requests which cannot be met, or of conditions which are not safe. Provides services when scheduled or communicates promptly to Agency and client any schedule changes. Completes and submits required paperwork in accordance with standard operating procedures. Represents Agency in a professional manner and dresses appropriate to job duties. Assists the agency in the event of an emergency. Performs other related job duties as assigned.

QUALIFICATIONS Education and Experience: High school diploma or general education diploma (GED); or an equivalent combination of education and experience.

Special Qualifications: Possession of a valid, State of Colorado driver s license. Requirement exists at the time of hire and as a condition of continued employment. First aid certificates or ability to obtain within three months of employment. Current PPD skin test results upon hire required. The employee is required to travel frequently throughout the County. Must be able to pass background check.

Salary $17.09- $25.86 per hour (hiring rate DOE)

Knowledge, Skills and Abilities: Knowledge of homemaking and personal care skills. Knowledge of, and can respond appropriately to, a variety of socio-economic and ethnic backgrounds. Skill in basic computer usage. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to write routine reports and correspondence. Ability to speak effectively to clients' and employees. Ability to add, subtract, multiply and divide, using whole numbers, common fractions and decimals. Ability to improvise in an unstructured home setting. Ability to use independent judgment to manage and impart confidential information. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to define problems, collect data, establish facts and draw conclusions. Ability to interpret a variety of instructions, written or oral. Ability to establish and maintain effective working relationships with clients, employees, the public, supervisors and other County officials.

PHYSICAL DEMANDS The work is medium work which requires exerting up to 50 pounds of force occasionally, and/ r up to 30 pounds of force frequently, and/or up to 10 pounds of force to move objects. Additionally, this position also requires the ability to frequently sit; use hands and fingers, handle, or feel; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; talk and hear; and taste or smell.

WORK ENVIRONMENT The noise level in the work environment varies. The work environment is highly variable and care is performed in private homes. The employee is regularly exposed to wet and/or humid conditions, moving mechanical parts, fumes or airborne particles, and risk of electrical shock. The employee is occasional exposed to high, precarious places; toxic or caustic chemicals; extreme cold; and extreme heat. Lighting, acoustics, air quality, temperature, and environmental hazards are not predictable.
