As the Care Management Supervisor, you will provide leadership and direction on the day-to-day operations of the department coordinating office resources and personnel, and maintaining and creating organizational programs to support the goals of Hawai‘i Health Partners (HHP). You will also be responsible for developing, implementing, and maintaining guidelines to facilitate the operations of the Complex Care Department and ensures quality customer service to patients, providers, and their staff. In addition, you will ensure the coordination of team efforts and effective communication and also performing duties and functions of Care management Services Registered Nurse. We are looking for someone dynamic and dedicated, with great organizational and communication skills and a commitment to creating a healthier Hawai‘i.

Minimum Qualifications: Current Hawai‘i Registered Nurse (RN) license or current Hawai‘i Licensed Clinical Social Worker (LCSW) license. Bachelor's Degree in Nursing, Social Work or related field. Current American Heart Association Basic Life Support (BLS) Healthcare Provider card. Two (2) years of experience in case management with experience working within a collaborative interdisciplinary team. Current driver’s license and valid Hawai‘i auto insurance with short notice access to an automobile to conduct site visits, as required.

Preferred Qualifications: Current Case Management certification. Care Management experience. Experience as participant in continuous quality improvement. Supervisory experience.
