Don't miss out on this chance to make a difference in the lives of seniors - apply now and take advantage of our excellent benefits package!   DESCRIPTION OF WORK The primary responsibility of this role is to engage with older adults starting from their first interaction, as well as their families and professionals, and provide ongoing care management support. This includes carrying out specialized tasks such as protective services, care transitions, and person-centered counseling. In this position, you will be responsible for managing a caseload of consumers and providing assistance in various areas as required. Additionally, you may need to coordinate services with physicians, community agencies, financial institutions, and other relevant stakeholders on a regular basis.

Work Schedule and Additional Information:

Full-time employment Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch. On-call hours when required. Salary:In some cases, the starting salary may be non-negotiable. QUALIFICATIONS Minimum Experience and Training Requirements: Three years as an Aging Case Aide 2; or A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training. Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

Other Requirements: Hiring preference for this vacancy will be given to candidates who live within Lackawanna, Monroe, Pike, Susquehanna, or Wayne counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered. You must meet the PA residency requirement. Legal Requirements: A conditional offer of employment will require a drug screening. You must pass a background investigation. This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
