Clinical Manager -PACT Program, Holyoke Job Title: Clinical Manager -PACT Program Location: 1727 Northampton Street Holyoke, MA, 01040 Employee Type: Full-Time Required Degree: Masters Minimum Experience: 2 Years Contact information Name - Jacob Lechner Email - jacob.lechner@bhninc.org Description Join our team of competent and compassionate professionals in the PACT (Program for Assertive Community Treatment) program at Behavioral Health Network! Maintains communication with funding sources or other contacts, as appropriate Ensures that the program adheres to regulations and requirements of licensing bodies and funding sources. Serves as primary liaison with other BHN programs. Directs and coordinates the client admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Schedules the admission interview; develops and coordinates the initial assessment and initial treatment plan; assigns the most appropriate staff to the individual treatment team (ITT); and provide clinical supervision of the development of the comprehensive assessment and the treatment plan for each client. The Program for Assertive Community Treatment (PACT) program is a highly integrated outreach and 24/7 residential program that helps five individuals meet their goals and reduce hospitalizations. PACT provides clinical wraparound support in the community and in our group residential program to perpetuate recovery and wellness for the individuals served. The PACT team is forensic-focused and designed to reduce arrests and incarcerations. Qualifications: Master\'s Degree in Social Work, Psychology, or Mental Health Counseling (required) Minimum of 2 years in human services (preferred) Individuals must be committed to empowerment and growth, human rights, and community integration of individuals served. Demonstrates willingness and ability to treat challenging and diverse client populations, including a forensic population. Demonstrates cultural competence in supporting clients with a variety of different cultural backgrounds. Demonstrates working knowledge of community resources, and how to help clients access them. Demonstrates willingness to work with individuals who are forensically involved within the Sequential Intercept model. Must have valid driver\'s license and a reliable car available for working hours to transport individuals served.
