Description

Care Integration Coordinator will assist in the gathering of data for reporting clinical quality measures. Data gathering will incorporate patient charts, patient interviews, and workflow design for provider offices. Duties may also include performing selected procedures and educational follow up with patients. Care Integration Coordinator work collaboratively with physicians, staff and other health care professionals to provide evaluation and assessment of patient needs. Duties involve both direct (in person) and indirect (via phone or online) patient care activities. They must demonstrate autonomy and decision making consistent with the values and ethical guidelines of the health system as well as the professional code of ethics. Care Integration Coordinator must also perform with minimal supervision.

Required qualifications:

• 1 year of experience with customer service, patient education and or patient engagement.

Preferred qualifications:

• Graduate of an accredited licensed vocational school of nursing, certification from a community health worker program, or accredited program for medical assistants.

• 1 year if experience in a health care facility.
