Fairview Health Services Visit Facilitator Specialty in Hibbing, Minnesota

Fairview Range is hiring a Visit Facilitator to join our specialty care team at the Fairview Mesaba Clinic in Hibbing, MN.

The Visit Facilitator is responsible for supporting in person clinic visits, including capturing vital signs, performing standard rooming, and other duties as assigned during the patient visit. This role captures pertinent medical information from the patient and documents the information into the patient’s medical record. This position also helps to ensure the exam rooms are stocked, equipment and supplies are available and assists with tasks that support the effectiveness and efficiency of the team.

We provide high quality health care for the entire family from newborn to geriatric, with the full range of primary and specialty care services. With clinics located in Hibbing, Mountain Iron, and Nashwauk, we provide convenient and easy access to our care services. Through our relationship with M Health Fairview, your local primary care provider can extend direct access to the physician specialists and advanced services offered by University of Minnesota Physicians to you and your family.

Schedule: This position is part time, 32-59 hours per two weeks, 8-hour Day/Evening shifts and weekends as needed. This position will be primarily based in Hibbing but may provide coverage in Nashwauk and Mountain Iron as needed. This position will primarily work in Urology, but may help cover other specialty areas as needed.

As a Visit Facilitator, you will be responsible for:

  • Preparing patients for their appointments including communicating with patients in advance of their scheduled appointment as needed to ensure medical records are collected, received and available for visit if applicable.

  • Assisting in the rooming process at the time of the in person visit by collecting and documenting pertinent information such as the reason for their visit and applicable health questionnaires.

  • Capturing vital signs via automatic vital sign machine, gathers patient information via questionnaires and questions populated in patient health record, helping guide patients through the care experience.

  • Performing EKGs and spirometry and sets up sterile field for procedures.

  • Preparing exam rooms and stock supplies.

  • Supporting the patient connection to the provider for the visit and serves as a resource ensuring a positive patient/provider experience.

  • Completing visit wrap-up ensuring the after-visit summary is provided and patients are aware of recommendations for follow up. This may include assisting with scheduling and coordinating follow-up appointments.

Required Qualifications:

  • High school diploma or equivalent.

  • 1 year customer service experience.

Preferred Qualifications:

  • Successful completion of Nursing Assistant, Emergency Medical Technician, and/or Health Unit Coordinator program.

  • Healthcare and customer service experience.
