Mental Health Association of Nassau County Nurse Practitioner in Hempstead, New York

PROS Employment Status: Part Time Finger Printing Required: Yes Driving: Yes Responsible for providing care, supervision, education, health assessments and monitoring, to adults diagnosed with mental illness participating in the Gathering Place PROS Program. The Nurse Practitioner will ensure that all medical/nursing policies and procedures are carried out and that the care provided promotes maximum overall health and mental health and is consistent with members personal recovery goals, cultural background and beliefs. Must be able to physically intervene in crisis situations as needed. Must be able to handle simultaneous challenging situations in an often-high stress environment. Must have flexibility, personal integrity, and the ability to work effectively with members, personnel and support agencies. Able to think objectively in various situations. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Sits, stands, bends, lifts and moves intermittently during working hours. Is involved with members, staff, visitors, etc. under all conditions and circumstances. Prescribe psychiatric medications under the supervision of the PROS Psychiatrist. Knowledge about the symptoms and treatment recommendations for mental illnesses. Knowledge of practices and procedures as well as laws, regulations and guidelines that pertain to community mental health programs. Responsible for providing health and risk assessments and facilitating referrals to community medical doctors and treatment in collaboration with treatment team and members desires with the goal toward better health management. Mental Health Association of Nassau County, Inc. Provide education to members and staff on medical conditions, potential health risk factors, medications, preventive/post measures to manage medical conditions and psychiatric symptoms. Conduct group training for members in habits of personal hygiene, disease prevention, health and wellness, and proper usage of medications. Conduct necessary training and in-service for staff. Assist with other health-related duties as necessary and required. Participates in the referral, admission and discharge process of members in the program as needed. Assists PROS staff in the development of initial plans for members who are accepted and treatment alternatives for individuals not accepted into the program. Ensures all nursing and psychiatric policies and procedures are carried out within Federal, State and Agency guidelines. Follow Universal Precautions and establish safety regulations in the use of equipment and supplies. Participate in maintaining the PROS Clinic Office in a clean and sanitary manner. Follow procedures for reporting and correcting unsafe or hazardous conditions/equipment. Completes all documentation and reports in a timely fashion and as required. Participate in the implementation of agencys Quality Improvement Program. Attending staff meetings. Develop and maintain a good working rapport with all staff to assure services and activities are adequately maintained to meet the needs of the members. Identify and assist in correcting problem areas, and/or the improvement of services. Participates in all fire safety and disaster preparedness drills in a safe and professional manner. Accessible after program/clinic hours to on-call staff for consultation and support for member emergencies. Participates in supervision with the Psychiatrist as requested. Accepts and follows through on other duties as assigned. Conduct and/or coordinate all infection control, and other health-related training. Maintain professional status through in-service and other trainings and participate in continuing educational programs designed to assure knowledge of changes in profession.
