Eliot Community Human Services Brain Injury Res Counselor in Haverhill, Massachusetts
Description
Eliot is seeking a candidate who has the ability to advocate for community supports and services on behalf of the individuals. and is able to identify effective strategies to address clinical issues/events.
Eliot Supports and empowers survivors of brain injury with individualized community based services in both living and day program environments. Our programs offer services designed to rebuild communication, social and daily living skills that increase independence and enhance the quality of life.
Responsibilities:
Assist with the day to day operations of a group home for individuals with developmental disabilities and/or brain injury.
Provide a safe living environment for all individuals, implement crisis intervention as necessary.
Participate in the implementation of Individual Support Plans (ISP).
Provide support with ADLs and other physical needs of individuals being served
Write/ maintain all documentation, related to individuals’ treatment plans and program operations in an organized and timely manner.
Assist individuals in being fully integrated in their communities by supporting them in the use of community resources, encouraging them in establishing memberships within their communities, and reducing stigma within the community through education..
Actively take part in supervision, staff meetings, in-services and trainings
Transport individuals to day programs, appointments, and activities as needed.
Schedule:
Sunday 5p-11p, Monday-Thursday 3p-11p (38hrs)
Sun 9a-11p, Friday/Saturday 3p-11p (30hrs)
Saturday/Sunday 1p-9p (16hrs)
