Trinity Health Throughput Coordinator RN Weekends in Hartford, Connecticut

Description:

Position Purpose : As the Throughput Coordinator (RN) at Saint Francis Hospital, your mission is to facilitate efficient patient flow and transitions to the next level of care. You’ll collaborate with healthcare professionals to enhance throughput and ensure seamless movement within the hospital system.

What You Will Do :

Patient Flow Optimization :

  • Identify and address barriers to discharge.

  • Prioritize discharge-ready patients.

  • Track metrics and report data trends for process improvement.

Effective Communication :

  • Communicate clearly with administrators, colleagues, patients, and family members.

Technical Proficiency :

  • Utilize computer-based applications (E-MAR, clinical documentation) and standard tools like Microsoft Office.

Team Collaboration :

  • Work closely with interdisciplinary care team members, including logistics, floor staff, and nursing unit leadership.

Minimum Qualifications :

  • Education : Graduate of an accredited school of nursing.

  • Licensure : Current licensure as a registered nurse (RN) in Connecticut.

  • Experience : Four (4) years of throughput coordination or case management experience in an acute care setting.

Position Highlights and Benefits :

  • Weekend-only position (Saturdays and Sundays).

  • 12-hour shifts (7 am – 7 pm or 8 am – 8 pm).
