Reese Family Services Program Director - In Home Supports in Hampton, Virginia

Job Summary: We are seeking a dedicated Program Director to oversee and manage our program operations. The ideal candidate will have experience in program management, and supervising staff, particularly in working with individuals with developmental disabilities. The Program Director will be responsible for developing, implementing, and evaluating programs that support the behavioral health and well-being of our clients. The Program Director will maintain the day-to-day operation of the in-home support program. conducts and/or reviews performance evaluations of staff in accordance with policy; monitors staff performance on regular basis and provides feedback; meets regularly with facility directors and staff; communicates regularly with the community; monitors equipment and supplies; reviews curriculum and service delivery model; evaluates individuals referred and receiving services; monitors implementation of programs; coordinates and monitors the documentation process; orientates, trains and supervises DSPs and other residential coordinators; prepares staffing reports; ensures implementation of Individual Service Plan; participates in Interdisciplinary Team meetings; assesses and evaluates individuals strengths and needs; advocates for individuals, make program modifications as needed; reviews, signs, and date program data collection process; coordinate all services offered to each individual; ensures individual exhibit signs of good personal hygiene; investigates individual and staff incidents and complete required documentation; ensures compliance with individuals' rights; ensures individual ratio is maintained as established by residence; participates in discharge planning; monitors behavioral management.

Duties: The Program Director/Quality Assurance Director is responsible for and leads the identification of facility-wide risk. The Program Director's role is to establish within its program safety guidelines that address activities related to health, safety, and the welfare of individuals receiving services, employees, contractors, and visitors. The Director also manages insurance procurement, develops and implements risk management and compliance policies/procedures, and manages claims and litigations alongside the Executive Director.

  • Oversees and monitors program risk management development and operations
  • Prepares and implements facility Program Best Practices
  • Establish and complete the facilities' monthly, quarterly, and annual Self-assessment monitoring documentation.
  • Completes Internal Incident Reporting Form; Uploads to CHRIS and Huma Rights the required reports with classification in the required reporting timeframe
  • Completes and forwards the Root Cause Analysis
  • Ensure serious incident reporting protocols with specific levels are reported, and documented, through CHRIS and to Human Rights.
  • Quarterly Reports and Annual Reports for Risk Management
  • Ensure Confidentiality and Security of Individual and Personnel Records
  • Ensures effective and quality services
  • Ensures Emergency Preparedness Policies and Procedures are current
  • Ensures staff, contractors, adhere to policies #440, #520, and #530
  • Attends workshops, seminars, and conferences to stay abreast of quality assurance updates

Position Type/Hours of Work

This is a full-time position, 40 hours per week in the office, overtime may be required from time to time. The employee may be required to accept 24/7 on-call responsibilities due to the dynamics of the position.

Regularly required to stand, walk, sit, use hands to type, handle objects, tools, or controls; and reach with hands and arms. Regularly required to talk and hear. Must be proficient in written and spoken English and have the ability to hear w ispers with or without the aid of a hearing aid.

The employee may be exposed to viruses, diseases, and infections from individuals receiving services and specimens in the working environment.

The employee may witness traumatic situations including physical assaults by individuals receiving services on other individuals receiving services or staff, psychiatric decompensation, and death of individuals receiving services. May be required to lift or move up to 50 pounds occasionally.

Work Authorization Clearance

All employees must provide a valid export document. Acceptable documents include a U.S. Birth Certificate, Certificate of Naturalization, Passport, Passport Card, Employment Authorization Card, or Permanent Resident Card. The successful completion of criminal history and central registry background investigations is required.
