Community Clinic Inc. Sexual and Reproductive Health Clinical Nurse in Greenway, Maryland

KEY FUNCTIONS AND RESPONSIBILITIES

  • Performs routine nursing care, care coordination and management for CCI’s primary care and sexual and reproductive health programs.  

  • Provides sexual and reproductive health services – including providing evidence-based patient education, conducting comprehensive sexual histories, STI screening and providing treatment per CCI standing orders. 

  • Assesses patients within the parameters of their licensure in collaboration with clinicians to identify a patient’s healthcare needs and care gaps.  

  • Adheres to evidence-based care practice guidelines and CCI standards of care.  

  • Maintains a panel of patients receiving PrEP – coordinating follow-up, labs, prescriptions and or administration of injectable PrEP per CCI standing orders.  

  • Reviews and reconciles medical and vaccine records.  

  • Manages a panel of patients, developing care plans, assessing barriers to care, coordinating transitions of care and supporting the referral process – including utilizing motivational interviewing principles to achieve improved outcomes.  

  • Executes treatment plans and evaluates results including providing point of care testing, vaccination and medication administration, refilling patient prescriptions and obtaining prior authorizations. 

  • Attends and participates in meetings, committees, health fairs, events and trainings as assigned.  

  • Participates in quality assurance, quality improvement, data reporting and workflow analysis projects as assigned. 

  • Trains new employees and participates in ongoing training of medical assistants and other support staff. 

  • Consistently seeks avenues for self-education to maintain compliance with evidence-based practice and advancements in sexual and reproductive healthcare.  

  • Maintains the patient’s electronic medical record within expected timeframes, following CCI standards for charting. 

  • Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive and trauma-informed manner.  

  • Works within the policies and procedures of CCI Health Services Inc. 

  • Performs other duties responsibilities as assigned by the Director of Nursing or designee. 

    MINIMUM QUALIFICATIONS: 

  • Active RN or LPN (RN preferred) license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.  

  • Current BLS/CPR certification required. 

  • Sexual and reproductive health experience or other infectious disease experience preferred.  

  • Written and oral fluency in English. 

  • Working on-site is an essential duty for the job – with opportunity for a limited hybrid work schedule.  

  • This position may require travel between CCI sites as needed.

  • Work under stressful conditions within a controlled, enclosed environment daily.  Work environment involves exposure to potentially dangerous materials and situations that require extensive safety precautions and may include the use of protective equipment.
