In accordance with the mission and goals established by the hospital and department management, provides nutritional care for ambulatory clinic patients, contracted services and outpatients of all ages. Assist with responsibilities in areas of projects, performance improvement, competency checks, peer review, development of policies and procedures and other nutrition related activities.

Essential Job Functions

  • Completes nutrition screening, assessment and reassessment of patients

  • Collects patient health assessment data on admission and in an ongoing systematic manner, focusing on the physical, psychosocial, age, cultural and spiritual needs of the patient.

  • Documents the initial assessment on appropriate medical records within the established time frames.

  • Reviews medical records, collects relevant information, identify conditions that effect nutritional status, interpret lab data, and food-drug interactions.

  • Conducts patient interview and obtains diet history and assesses nutritional adequacy.

  • Utilizes appropriate tools to evaluate nutritional status and progress.

  • Involves the patient/family and other healthcare providers when appropriate.

  • Complies with completeness, accuracy and timeliness of documentation.

  • Determines plan for reassessment based on protocol process or patients’ needs/status.

  • Conducts follow-up for patient satisfaction.

  • Develops an educational process for the provision and coordination of patient care activities that promote and maintain health, fosters self-care and improves outcomes.

  • Evaluates, creates and revises education material related to disease management and current diabetes topics.

  • Develops and conducts education programs for staff and community.

  • Participates in the interdisciplinary education to assess the patient's readiness to learn, identify the patient's educational barriers and tailor the educational processes to meet the needs of the patient/family.

  • Maintains a collaborative relationship with physicians and other health care providers to achieve desired patient outcomes and assure a continuous flow of patient care.

  • Communicates pertinent information for interdisciplinary patient care conferences.

  • Documents all aspects of diabetes care including assessment, plan of care, response to intervention, progress, and education provided to patient, utilizing acceptable format, standards and guidelines established by the system and according to the regulatory agencies guidelines and requirements.

  • Produces documentation that is accurate, concise, timely and legible and which reflects an interdisciplinary plan of care.

  • Post charges in a timely manner for outpatient services rendered.

  • Attends appropriate operation meeting.

  • Works collaboratively with other team members.

  • Participates on interdisciplinary committees.

  • Involves all appropriate departments in the development of policy/procedures or changes.

  • Compliance to Regulatory Agencies

  • Assists on preparation for surveys/inspections.

  • Responds appropriately in emergency situations.

  • Assists in maintaining a safe and clean environment for patients, visitors and staff.

  • Assists in maintaining manuals of meeting notes, communications, policies and procedures and performance improvement initiatives.

  • Adheres to the infection control processes, including standard precautions, to reduce the risk of acquiring or transmitting infections.

  • Provides Outpatient Nutrition Counseling

  • Includes the patient in determining their health goals to incorporate proper nutrition into their lifestyle.

  • Establishes short- and long-range goals with the patient that assists them in adapting to and complying with recommended nutrition practices.

  • Identifies and uses the most appropriate/effective counseling method for the patient.

  • Incorporates principles of proper nutrition, weight control and exercise in provided education.

  • Accurately documents all counseling interactions.

  • Provides feedback to referring physician.

  • Obtains physician referrals as required.

  • Participates as an interdisciplinary member in Well Being Center activities.

  • Actively participates in the development and implementation of health education and wellness programs.

  • Works collaboratively with other team members in program presentation to employees, employers and the community.

  • Participates in Well Being and Excela Heath marketing activities.

  • Provides Health Coaching

  • Provides health coaching to individuals utilizing the Wellness Mapping 360 model.

  • Accurately documents all coaching interactions

  • Measures and documents outcomes at end of coaching relationship.

  • Provides Biometric Screenings

  • Provides biometric screening including weight/BMI, blood pressure, waist circumference and finger stick lab testing.

  • Accurately enters all data into the MyBioCheck application according to the Biometric Screening Policy/Procedure.

  • Reviews results with participant and provides brief counseling.

  • Other duties as assigned.

Competency and Experience [Knowledge, Skills and Abilities]

Required Qualifications/Experience

  • Bachelor’s degree in Nutrition, Food and Nutrition, closely related field or higher level of education in related fields

  • Completion of an American Dietetic Association approved program leading to registration.

  • Six (6) to twelve (12) months of experience in functional area for this role.

  • Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment.

  • Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.

  • Demonstrate a thorough knowledge of modified diets and enteral/parenteral feedings; professional nutrition planning and instructional skills and knowledge of diabetes care and management.

  • Ability to use Microsoft Word, Outlook, and Power Point.

Preferred Qualifications/Experience

  • Experience in Healthcare setting.

License, Certification & Clearances

  • Current License to practice in the State of Pennsylvania or within 6 months of hire.

  • Current registration with the American Dietetic Association or within 6 months of hire.

  • Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program required.

  • Act 33 with renewal

  • Act 34 with renewal

  • Act 73 FBI Clearance with renewal.

Position Type/Expected Hours of Work

  • Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).

  • Travel may be expected locally between System locations.
