The Home Options Path to Empowerment (H.O.P.E) Program is a nursing home transition program and assists individuals move into their own home in the community and supports our members so they are safe, comfortable and happy.

This innovative program uses interdisciplinary teams of social workers, care managers, nurses, occupational therapists and social services practitioners to assist members to transition to community living.

Be Part of Building a Program-- this key leadership roles is responsible for overseeing the Transition Team (nurses, social workers and occupational therapists). This Transition team is responsible for using their expertise and objectivity to build skills and supports our clients will need to live in the community. Actively provide leadership, guidance, support and accountability to the staff as they work to to ensure members are transitioned into the community seamlessly and safely.

What are some of the specifics?:

  • Lead, grow and develop the interdisciplinary team. Foster perspectives as they relate to integrated aspects of our members; Bring those perspectives together to complete steps to a transition and care and support plan for our members

  • Ensure all benefits and services are arranged and transferred upon transition

  • Lead our efforts to maintain successful goals and achieve established metrics.

  • Point of contact for governmental authorities or members for escalated concerns

  • Collaborate with the other interdisciplinary teams (Service Plan, Pre-Transition, Operations and Community Integration) to ensure program compliance with policies, procedures and contracts. Work within the H.O.P.E. team to perform Root-Cause Analysis to troubleshoot for problems and make recommendations for changes. Work across the program in establishing best practices, fostering a can-do approach and achieving goals.

  • Deliver training, performance feedback and developmental coaching.

Required Experience:

Who are We Looking For? Someone With:

  • B.S. Degree in Nursing

  • M.S. Degree in Nursing, Social Work, MBA or related field. RN or LCSW licensing strongly preferred

  • at least 5 years of successful proven experience providing social services in a hospital, rehabilitation or mental health environment; Assisted Living Community; Managed Care Organization or other similar healthcare environment

  • At least 3 years of supervisory or management experience to include coaching and developing employees

  • Advanced technical, analytical, organization and interpersonal skills

  • Ability to manage multiple tasks simultaneously while working with others autonomously

  • Strong tech savviness

  • A strong can do approach and ability to hone in on key strategies and implement best practices
