WorkSource Oregon Pre-Exposure Prophylaxis (PrEP) Coordinator in GRANTS PASS, Oregon
Bilingual candidates are eligible for an additional \$1.50 Bilingual Differential. This bilingual differential is not included in the salary above. Position Summary The Pre-Exposure Prophylaxis (PrEP) Coordinator works directly with populations that are at high-risk for acquiring HIV, to assist them in accessing Pre-Exposure Prophylaxis, or PrEP. The PrEP Coordinator works closely with HIV Alliance\'s STI testing clinic, the Testing Coordinator and Early Intervention Specialist to identify and reach out to high risk individuals. The PrEP Coordinator provides education and information to individuals interested in accessing PrEP, provides referrals to prescribing physicians, and works with the individual to identify and overcome barriers to accessing PrEP. The PrEP Coordinator assists clients in accessing and enrolling in insurance programs, including but not limited to Oregon Health Plan (Medicaid), Medicare, and private insurance plans. This position provides HIV testing services, community education and conducts outreach to high risk populations. Essential Functions and Responsibilities Client identification, outreach and engagement Provide direct one-on-one and phone PrEP coordination services Visit accompaniment services Coordinating access to the on-site PrEP clinic to include scheduling clients in EHR system, scheduling follow-up appointments, and sending out consents, ROIs, and other paperwork for clinic access. Provide appropriate referrals, including follow up and documentation Advocate with medical providers as needed on behalf of our clients Provide HIV testing Maintain up-to-date HIV and PrEP knowledge and provide HIV and PrEP education to high-risk populations, professionals and community members, in a variety of settings Maintain up to date pool of referrals for clients in our service area Develop and strengthen relationships with service providers in our service areas Maintain up to date information regarding prescription assistance programs Participate in necessary meetings Assist Director with all necessary reports Other duties as defined by Director and program standards Conduct outreach online, in community sex environments, and at LGBT-related events to gay and bisexual men, offering safer sex supplies and making referrals to our services Assist Prevention Manager in planning community events For a complete job description, please visit: https://hivalliance.criterionhcm.com/jobs/51/1/#614
