Syringa Hospital & Clinics is accepting applications for a Full-Time Discharge Planning Registered Nurse. The Discharge Planner is responsible for facilitating continuity of health care for each admitted patient through the discharge planning process. Current RN licensure, or eligible, with the Idaho State Board of Nursing; or Licensure (BSW) in social work in State of Idaho or Bachelors or Associates Degree in related field with experience in healthcare field.
