Corewell Health Physician - Medical Director for Clinical and Social Care - Grand Rapids, MI in Grand Rapids, Michigan

Corewell Health is seeking to fill the position of Medical Director for Clinical & Social Care. The incumbent will collaborate with the Vice President for Health Equity Strategies in embedding health equity into all parts of the organization by integrating public health principles into healthcare. Key activities will include communicating, socializing, and operationalizing an equity-promoting model for integrating clinical and social care; providing technical assistance and other kinds of support to clinical service lines seeking to integrate equity into their work; and serving as an equity ambassador, thought leader, communicator, and co-strategist in collaboration with clinical service lines.This role will also work closely with the VP Community Investments by helping to: amend, draft and approve policies for the School Nurse Program; create processes, procedures and practices that support clinical policies within the School Nurse and Chronic Disease management programs; and provides clinical consultation, clinical subject matter expertise and supports/services to community stakeholders.In addition to working closely with the VP, Health Equity Strategies the VP, Community Investments, this position will also collaborate with the Medical Director, Equitable Outcomes on the Quality Safety and Experience team.The ideal candidate has competencies and experience in both healthcare and public health.

Essential Functions:

  • Health Equity- Provide technical support, guidance and advice to clinical service lines seeking to integrate equity into their work;- Serve as a physician champion for health equity across the system, including supporting clinical and operational processes, procedures, programs, policy, and other interventions to advance equity of care;- Attend provider meetings. Huddles, etc. to stay abreast of priorities, concerns, activities, etc. and ensure alignment with enterprise strategy/equity-promoting integrated model for clinical and social care.- Collaborate in ideation, writing, co-lead (as PI or co-PI), and executing grant proposals;- Conduct internal and external presentations (including CMEs); and- Collaborate with Communications and Marketing Teams in creating and disseminating messaging related to the system health equity strategy and the transformation model (i.e., equity-promoting integrated model for clinical and social care - a new model for care delivery).

  • Community Health- Provide clinical support, guidance, oversight, and consultation, as needed, in clinical settings and to community programs including (but not exhaustive): Review, craft and sign off on clinical policies, procedures, guidelines, and clinical pathways, etc.;- Serve as a link, translator, and ambassador between community health and clinical stakeholders to ensure adherence to policies and standards of practice;- Review clinical end points; sign off on billed services and nurses’ standing orders;- Bring latest research and determine best practices & process for clinical decision making and program development;- Serve as PI on grants, as needed;- Sign contracts, as needed, and provide oversight of CLIA-waived lab testing.

Qualifications:

  • Required Doctorate MD or DO

  • Preferred Master's Degree MPH or DrPH

  • LIC-Physician (MD) - STATE_MI State of Michigan required Or

  • LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan required
