Job Title: Bilingual Social Worker -- Senior Services Program

Location:  Grand Rapids, MI Rate of Pay: $25.32 per hour Shift: 8:00 am -- 4:30 pm (includes a 30-minute unpaid lunch)

About the Position: Our client is seeking a dedicated and compassionate Bilingual Social Worker to join our Senior Services program. This entry-level social work case management position focuses on supporting adults over the age of 60 by providing essential services that enhance their quality of life.

Primary Responsibilities:

  • Conduct needs assessments and screenings to determine client needs.
  • Develop personalized service plans, including setting goals, objectives, and actionable steps.
  • Coordinate referrals to appropriate services and resources.
  • Monitor and track clients' progress toward goal completion, including conducting follow-ups.
  • Assist clients with benefits applications and navigate the application process.
  • Provide written and verbal translation services as needed.
  • Advocate for clients to ensure they receive the support and services they need.
  • Deliver services through various methods, including in-person office visits, home visits, phone calls, and community outreach.

Qualifications:

  • Bachelor's degree from an accredited institution in sociology, psychology, behavioral sciences, human services, or a related field.
  • A minimum of two years' experience as a social worker, case worker, or project manager. Candidates with an equivalent combination of education and experience will also be considered.
  • Fluency in Spanish, including the ability to read, write, speak, and communicate effectively.
  • A valid Michigan driver's license is required.
