Licensed Mental Health Counselor - Admissions Case Manager
Duties and responsibilities include but are not limited to: Assesses the psychosocial functioning and needs of the Veteran, collaboratively working with their family members identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. Provides case management and care coordination to facilitate appropriate delivery of health care services and incorporates complex multiple causation in differential diagnosis and treatment of Veterans. Collaborates with the internal teams to develop discharge plans and set achievable discharge goals with consideration of the Veteran's personalized recovery goals. Makes referrals to other services internally and with other agencies, community resources, and other governmental agencies as appropriate.
Requirements: United States Citizenship Current, full and unrestricted Mental Health Counselor license from any US state or District of Columbia Masters degree or better from a program accredited by the Council on Accreditation of Counseling & Related Educational Programs (CACREP)

The facility consists of 53-bed capabilities for surgical procedures, specialty care, mental health in/outpatient services, as well as a Community Living Center, and social programs and services.
