Description:

This position is responsible for assessment, treatment and case management to Trinity Health patients. The position assists in the collaboration and implementation of a plan of care for the patient. This position provides care coordination and discharge planning.

Position Purpose : Case Manager LMSW I to work in the Case Management Department at Grand Haven Hospital.

What you will need:

  • Masters degree in social work required

  • Prior employment experience in a related area preferred

  • State of Michigan, social work license required

What you will do:

  • Identify, coordinate communicate and participate in discharge planning with patients and a multidisciplinary team

  • Recognize and monitor potential barriers to safe discharge planning and intervene as appropriate

  • Navigate community resources as well as serve as a reference and liaison for community resources and service.

  • Perform conflict resolution and crisis intervention when and where appropriate

  • Accurately document interventions, assessments, and discharge planning in the electronic medical record
