OVERVIEW OF PRIMARY RESPONSIBILITES :

MRT Peer Practitioner will be responsible with assessing and following up with all and linking to any behavioral health services that can assist with stabilization. MRT Peer Practitioner will need to utilize evidence base trainings and screening tools. MRT Peer Practitioner will collaborate with other providers to assist with referrals and linkages. MRT Peer Practitioner will provide face to face follow up with Clinician on prior calls that could benefit from behavioral health support.

An MRT Practitioner does not require a license, but must be able to provide professional staff (Licensed Clinicians) with back up duties within the Mobile Response Team. Practitioners are required to be skilled in the clinical triage and assessment of all emergency, urgent, and routine calls.

PRIMARY FUNCTIONS :

• Will respond to all calls to the MRT with base clinician.

• Will assist in assessing individual to provide them with the support needed to divert an ER visit or hospitalization.

• Will assess calls as a backup to the Base Clinician as needed and be able to assess an appropriate clinical response as authorized by the licensed clinician on the team.

• The practitioner must be skilled in developing exceptional follow-up diversion planning for individuals served by the Mobile team.

QUALIFICATIONS AND ATTRIBUTES :

  • Peer certification or willing to obtain

  • Strong organization skills with accurate attention to detail

  • Excellent communication skills and the ability to multi- task

    EDUCATION AND EXPERIENCE :

  • 1-3 yrs. experience in a Human Service related field and/or lived experience
