Case Manager, Assertive Community Action Team Reporting to: ACAT Program Director Salary: Commensurate with Education and Experience Hours: flexible, full-time, some on-call hours Location: Office in Frederiksted, work throughout St. Croix Do you have experience in homeless and human services and want to work for a team that actively improves the lives of the individuals it serves? Case Manager: This position is responsible for the delivery of comprehensive, intensive, clinical-based case management for adults and small families experiencing homelessness or housing vulnerability. The Assertive Community Action Treatment (ACAT) services program is facilitated by Liberty Place, Inc. Our services are based on the ACAT model as well as motivational interviewing, stages of change, strengths-based case management, housing first, and trauma informed care. The position will provide outreach services, referral and linkage with mainstream services and housing, and crisis intervention for program clients. This position helps ensure that support and programming needs are in alignment with the mission, vision, and values of Liberty Place, Inc. on St. Croix. Key Responsibilities and Duties: Provides strengths-based case management, specifically housing placement and support, to homeless individuals and disabilities including substance use and significant primary and mental health care needs. Works within an interdisciplinary team environment to enhance the clinical outcomes and supportive housing services. Assists with connection to resources including health, mental health, and substance use treatment. Advocates for the needs of individual clients as well as for systemic change, whenever needed for the overall benefit of homeless individuals. Participates in team meetings to coordinate activities with other team members and programs. Accepts referrals internally from the Housing Intake and Placement Team as well as externally from island allied agencies utilizing a Coordinated Entry process working collaboratively with staff of all agencies to provide high quality service. Works with primary clients, as well as team clients, related to service planning, documentation of progress, service tracking, and outcome evaluation. Monitors client progress with regular, ongoing contact with individuals receiving services in conjunction with partner agencies both external and internal. Serves as a liaison, when needed, with partner agencies and teams, establishing and maintaining effective working relationships with organizations and groups providing services to our target population. Assists with work that needs to be done at moments of urgency or crisis. Must be accessible when in the field to the ACAT Director, team, program clients, and community partners. Navigates the Social Security, Medicaid, and Medicare systems with clients. Completes Medicaid authorizations, HMIS registration form, data entry, as well as any other reporting requirements necessary to obtain and sustain funding and provide services for individuals served. Documents all services in a timely and complete fashion. Maintains up-to-date records. Documents telephone and other communications as needed. Manage tasks and scanning as needed to have a complete and current chart. Maintain accurate information regarding client's insurance status. Meets formally with the ACAT Director for ongoing individual and/or group supervision. Attends and participates in clinical and administrative trainings/meetings as deemed appropriate and necessary for the job. Maintains current and complete COVID-19 vaccination(s), as defined by the Center for Disease Control and Prevention and local health auth rities. Performs additional duties deemed necessary and appropriate by the Community Program Coordinator. Position Qualifications: Bachelor's degree in human services, social work, psychology, or related field preferred or equivalent experience 5 years of community field experience with homeless population preferred, or previous experience in a related direct human service field working with underserved populations preferred. Experience managing peer and group support programs Case management experience Crisis management experience Exceptional communication skills with an ability to establish trust and rapport quickly Have empathy and relationship skills Proficient in Spanish (not required, but preferred) Ability to quickly condense information from multiple sources and communicate it Well organized Attention to detail Thrives in situations of ambiguity Strong analytical and critical thinking skills Dependable and meets deadlines Be available to respond to on-call, as needed Flexibility to work occasional nights and weekends, as needed Microsoft 365 experience (not required but preferred) Expectations: Confidentiality agreement with Liberty Place.
