South Middlesex Opportunity Council (COC) Street Outreach Worker in Framingham, Massachusetts

Summary:

Provide street outreach assessments to assigned clients, a combination of those living outside and areas not meant for habitation. Target population include those who are living outside and those identified by Police Officers or other community providers as needing services. Work closely with other community-based providers to ensure immediate and long-term health and safety needs of the participants are addressed. Work to engage clients in Case Management and refer to appropriate region and mainstream benefits. Develop/monitor plan to assist in improved recovery, housing status and overall health. Travel throughout the west region is required.

Primary Responsibilities:

  • Provide street outreach and implement engagement strategy with all new clients referred to the program and conduct a comprehensive service needs assessment with each client.

  • Provide outreach at the Day Center as needed and according to a schedule. Attend meetings at both SMOC, COC, and The HUB when required.

  • Guarantee data is accurately collected and submitted in VESTA

  • Develop, implement, and monitor Individualized Service Plan with each participant.

  • Ensure coordination of service delivery on behalf of each client. Work in collaboration with all other service providers working with the participant.

  • Build working relationships with other area agencies.

  • Assist individuals with medical emergencies in finding detoxification services and appropriate treatment.

  • Keep accurate client records and provide monthly written updates including client updates, demographics, barriers and new resources, etc.

  • Assist individuals in accessing new resources and supports which will lead to safer choices and reduce risky behaviors that may include panhandling, prostitution, and substance use.

  • Engage all clients by understanding and addressing their needs whether within or outside the scope of work.

  • Attend & participate in engagement team meetings as requested and communicate effectively with client and staff in other areas.

  • Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements. Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.

  • Other duties assigned.

Knowledge and Skill Requirements:

  • Bachelor’s degree or three years’ experience in Human Services or related field.

  • Sensitivity to low - income people of diverse backgrounds.

  • Ability to work well as a member of a dynamic team.

  • Valid license and reliable transportation.

  • Assessment, advocacy and case management skills.

  • Flexible schedule required.

  • Bilingual preferred.

    Physical Requirements: Must be able to conduct street outreach which often may include long periods of walking and standing while speaking with clients.

    Working Conditions : Frequent travel throughout the Western region of Massachusetts is required. The Street Outreach Worker will work at multiple office spaces, such as, the Day Center in Framingham and the Satellite office in Franklin, as well as, 7 Bishop Street in Framingham. As part of the responsibilities of this position, the Street Outreach Worker will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
