Advocates Care Management Supervisor in Framingham, Massachusetts

Overview

Salary: $65,000

The Care Management Supervisorworks in conjunction with the Integrated Care Management Directorof Operationstoprovide focused leadership,supervisionand support for the care coordination staff. The Care Management Supervisor coordinates behavioral health and substance use disorder care management servicesin accordance withthe individualized care plan. Theywork collaboratively with the clinical team to manage or co-manage supports/services needed for individuals with complex behavioral health needs. As a member of the multidisciplinary team, the Care Management Supervisor provides consultation, education, and support to team members and provider networkregardingbehavioral health/substance abuse treatment and management.

Responsibilities

  • Hire, train, supervise and evaluate care coordination staff. Maintain consistent communication with staff and conduct regularly scheduled meetings with staff.

  • Monitor program documentation to ensure accuracy and timeliness.

  • Support theIntegrated Care Management Directorof Operationsin the management and administration of the assigned programs andmonitorand improve productivity.

  • Provide support to outreach,engageandassistclients inidentifyingtheir health/wellness and recovery goals and developing their individualized care plans.

  • Utilize effective, dignified, empowering and creative engagement strategies to ensure clients are at the center and lead in their care team.

  • Provide linkages to medical care, behavioral health services, substance use disorder programs and community resources and supports.

  • Provide assistancewith navigating the healthcare system and provide support/assistancewith transitions in care.

  • Collaborate with existing providers and other collaterals anddeliversservicesin accordance withmember’s plan of care. Facilitate communication and information exchange across the continuum of providers.

  • Coordinate all aspects of service delivery with team members, including ensuring access and securing authorization, asindicated.

  • Participate in team meetings to ensure effective communication among all disciplines involved inmember’s care.

  • Follow thememberacross the continuum of care for the purposes are care coordination.

  • Partner with the client, their care team providers and supports to ensure that care plan and safety plan are implemented as developed, and to make as needed adjustments.

  • Monitormember’s progress in treatment andassiststhe clinical team with evaluating the need for continued services.

  • Identifycommunity resources and develop naturalsupportsforclient.

  • Complete all required documentationin a timely mannermeeting all Agency and divisionestablisheddeadlines.

  • Attends and activelyparticipatesin supervision and staff meetings.

  • Attend assignedtrainings;maintainnecessary certifications and licenses (CPR/First Aid, Safety Care, MAP).

Qualifications

  • Master’s degree in social work, mental health counseling or related field with minimum of 2years experienceworking with adults in a community-based and/or medical setting, andMassachusetts independent practice licensure in Mental Health Counseling or Social Work.

  • One year of supervisory experience preferred.

  • Demonstrated knowledge base and judgment necessary for independent clinical decision making.

  • Excellent verbal and written communications skills. Strong interpersonal and customer relations skillsrequired.

  • Strong computer knowledge

  • Excellent organizational, time management andproblem solvingskills.

  • Ability to openly address and acknowledge issues of substance use and mental illness.

  • Knowledgeregardingpsychiatric rehab and understanding of recovery model.

  • Stronglyprefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.

  • Must hold a validdrivers’license. Must have access to an operational and insured vehicle and be willing to use it to transport members.
