POSITION: Behavioral Health Case Manager (BHCM)

EMPLOYMENT STATUS: Full-Time (FT)

Bilingual (Spanish and English) applicants are preferred, but not required.

WORK LOCATION : Travel between Fort Walton Beach (FWB) and Crestview (CV)

WORK HOURS: Mon - Fri; 8:00 a.m. - 5:00 p.m.

WAGE:

The wage for this position is $

20.00

per hour

(Bachelor's Degree) or $22.00 per hour (Master's Degree).

POSITION SUMMARY: Bridgeway Center, Inc. (BCI) is seeking applicants that want to work with clients to effectively address clients' care by assessing needs, developing, monitoring, and facilitating interdisciplinary approaches. This position is responsible for managing an assigned caseload of clients having mental illness diagnosis and for assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs. The Case Management department consists of services that help individuals link to needed medical, social, education, psychiatric, psychological, and other services. This includes direct assistance in gaining access to services, Coordination of Care (CoC), oversight of the entire case, and linkage to appropriate services. Case Management includes (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated service plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation. The Case Management department educates persons served and their support system regarding diagnosis, treatment, and recovery. This position will work with clients in the Judicial system.

MINIMUM REQUIREMENTS: Master's OR Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
