Responsibilities

Work Schedule:

Monday - Friday, between the hours of 7:00am and 5:00pm, to include a lunch break

No Weekends, No Holidays

Job Specific Position Duties: The duties include, but are not limited to the following:

• Initiate, perform and complete assigned duties in providing care to variable patient populations.

• Complete assigned patient care based on the patients’ conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report and record relevant patient information.

• Observe, identify and respond to the patient’s needs for care, including medication, equipment-assisted care and patient/family education.

• Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and perform treatments according to established policies/procedures.

• Recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.

• Perform support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient’s condition before, during, and following the procedure.

• Orient peers and ancillary personnel to unit policies and procedures, skills, and standards of practice.

• Integrate healthy work environment principles that result in improved patient healthcare delivery, education of patients and their families and employee satisfaction.

• Assist the Registered Nurse (RN) Case Managers and Licensed Clinical Social Workers (LCSWs) in the Case Management Department.

• Alert RN and/or LCSW case managers and physicians to significant changes or abnormalities in patients and provide information concerning their relevant condition, medical history and specialized treatment plan or protocol.

• Participate in multidisciplinary discharge planning and other professional staff meetings as indicated for complex patient cases and develop a database and knowledge of local community resources. Prepare routine reports and conduct analyses. Participate in video teleconferences (VTCs) and other meetings as required.

• Provide case management assistance with medically complex and/or catastrophic cases; and facilitate screening and assist with transfers of Exceptional Family Member Program (EFMP) families as well as other patients and families to ensure smoother transition and continued health care treatment when these beneficiaries transfer out of the area.

• Maintain patient records in an orderly, secure fashion. Document patient care and case management services in GENESIS and other software applications.

• Assist with the evaluation of patient, family and provider satisfaction and use of resources and services in a quality-conscious, cost-effective manner.

• Utilize Microsoft Word, Excel, Outlook, PowerPoint, and other software programs as well as computer literacy and typing skills to prepare typewritten documents, briefs, presentation materials, etc.

• Recognize conditions which require isolation. Ensure universal precautions are used in all patient encounters. Recognize emergency situations and institute emergency measures for sudden adverse developments.

• Participate in the design and implementation of activities to increase hospital staff involvement in and support of CM activities by providing orientation and ongoing education/in-service training specific to CM, the CM program, and available community resources.

Qualifications

Minimum Qualifications:

* Education: Associates Degree of Nursing

* Experience: Minimum 1 year experience, within the last 2 years; experiene must be in an ambulatory or outpatient care setting

* License: Active, unrestricted LPN/LVN license

* Board Certification: N/A

* Life Support Certifications: BLS

* Security: Must be able to pass a Government background check and obtain a Government security clearance.
