Flexible scheduling allows employees to work as many or as few shifts as they want as our team is looking for high-energy individuals looking for full or part-time work, nights, days, weekends and holidays.  Benefits  * Paid Time Off * Mileage Reimbursement * Free Training * Part of a team with RN Oversight * Employee referral bonus * Monthly CPR Class * Electronic charting * Flexible schedule * Employee of the month * HomeCare Pulse Employer of Choice * Weekly pay via direct deposit * Joint Commission accredited Responsibilities * Work cooperatively with physicians to develop an appropriate treatment and care plan specific to the patient and their physicians report * Assess the environment, especially as it related to fall risk and intervention and safety * Identify equipment needs and develop adaptive plan to improve patient function and independence * Report patient’s response to treatment or changes in condition to the Director of Nursing and physician * As appropriate, coordinate interventions with other health care providers and live setting such as child care center, preschool, school, worksite, adult day care or residential setting while instructing appropriate personnel, family and caregivers, on intervention goals * Prepare clinical and progress summaries as well as discharge planning. Submit documents to Director of Nursing * Treat patients, family, and staff with courtesy and respect Requirements * Graduated with a master's degree in physical therapy from an accredited physical therapy program * Successfully completed a period of supervised fieldwork experience required by the recognized educational institution where the applicant met the academic requirement of the educational program * State licensure in the state of South Carolina * Passed a National Physical Therapy Examination (NPTE) and/or state examination required by the state * Two (2) years of appropriate experience as an physical therapist, preferred community/home health experience * Adhere to HIPAA and maintain patient confidentiality We are an Equal Opportunity Employer and do not discriminate against applicants due to race, ethnicity, gender, veteran status, or on the basis of disability or any other federal, state or local protected class.
